Macedon Family Dental Savings Program helps patients and families without dental benefits get access to quality, trusted dental care. The Macedon Family Dental Savings Program is designed to help our patients and their families save money on periodic and preventive dental treatment and services.
MACEDON FAMILY DENTAL SAVINGS PROGRAM
ANNUAL MEMBERSHIP FEES:
INDIVIDUAL PLAN $375.00
SELF & SPOUSE $750.00
SELF & FAMILY $950.00
(Up to four family members maximum. Can include self, spouse and children under 25 years of age residing in the same household.)
MEMBERSHIP BENEFITS:
- Two teeth cleanings
- Two periodic exams
- Two fluoride treatments (for patients up to age 18)
- One set of full x-rays or Panorex
- One set of bitewing x-rays
*20% OFF ADDITIONAL SERVICES:
- Fillings, crowns & bridges
- Dentures & partial dentures
- Dental extractions
- Root canals
- Periodontal Scaling and Root Planing
- Emergency visits (not including after hours visits)

It is important to us that our patients understand the terms and conditions of the Macedon Family Dentistry Dental Savings Program (MFDDSP). MFDDSP is NOT dental insurance and cannot be combined with your existing plan. Covered benefits of MFDDSP are available only at Macedon Family Dentistry. Membership fees are non-refundable. Membership is valid for one year from date that the enrollment form and payment is received. Discounts do not apply to specialist or specialist referrals. Discounts cannot be applied to emergency afterhours care, dental veneers or implants, teeth whitening, implant crowns or bridges, Workers Compensation Claims or any type of orthodontic treatment as well as any specialist fees. Payment is expected at the time of service unless previous arrangements have been made. Fees may be paid by cash, check, credit card or FSA/HSA cards. Macedon Family Dentistry reserves the right to make changes to these terms and conditions at any time. Further restrictions may apply.
